Community Rehab Care provides Case Management
services to individuals in order to achieve the client's outcome goals.
Community Rehab Care's Case Management Staff have extensive experience in
neurologic rehabilitation. Our Case Managers are also approved by the State Wide
Head Injury Program.
To make a referral, please call us and clearly state that this is a referral for
Case Management Services. We will need some basic information initially including:
Client name, address, phone number, age, funding source and other relevant demographic information. We will then request some history of the client’s injury, and discuss with you the relevant issues, reason for the referral, and expected goals/outcomes of case management services.
Since case management services are generally not funded by
private insurance, financial arrangements will need to be made in advance
of client contact
SHIP case management referrals must all be pre-approved by
SHIP
Contact/Ongoing Communication:
We will maintain contact with the referring party/agency via phone, fax and written documentation based upon pre-determined and mutually agreeable time intervals.
