Community Outreach and Recreational Groups

Community Rehab Care assists in the transition from outpatient care to the community through skilled therapy. After Discharge from skilled therapy services, Community Rehab Care also provides community outreach though  Case Management services,  Recreational Groups, and a yearly Snowball event.

Over the years Community Rehab Care staff have been able to establish  Recreational Groups for Brain injury survivors and program clients. These groups are funded primarily through grants from the Statewide Head Injury Program, obtained by Community Rehab Care.

Community Rehab care employees also take an active role in various events and groups supporting individuals with various Neurological diagnosis; including Brain Injury and stroke. Staff assist to lead the B.A.B.I.S. group ( Boston acquired Brain Injury Support Group) and assist in planning the annual Heads Held High walk.

Northeast Recreation group Adults within the North Shore area who have sustained a Brain Injury Recreational program offering a variety of  activities. The last Thursday of every month Time: 6-8 pm* depending upon the activity Medford CRC or the destination of the month. Directions are always provided on the flyers. S.S.H.A.R.E.(South Shore Adventure and Recreational Entertainment) Adults within the South Shore area who have sustained a Brain Injury Recreational program offering a variety of  activities within the South Shore area Once per month, depends upon planned activity Where: Quincy CRC or the destination of the month.

S.O.B.I. (Survivors of Brain Injury)

Individuals ages 15-25 who have sustained a Brain Injury

Recreational program offered to adolescents to young adults ages Once per month, depends upon planned activity Out of the Newton clinic, however outings are in various community venues within the Boston area

Dinner Out Program Nursing Home Initiative clients who have been referred from their CM What: Recreational program to NHI clients Dinners are held 4 times a year Local places of interest which are Handicap accessible. Referral from Community Rehab Care Nursing Home Initiative Case Manager. For information Please Call 617-786-8811 and ask for information on the NHI dinner out Program
