Community Rehab Care assists in the transition from outpatient
care to the community through skilled therapy. After Discharge from skilled
therapy services, Community Rehab Care also provides community
outreach though Case Management services, Recreational Groups, and a yearly
Snowball event.
Over the years Community Rehab Care staff have been able to
establish Recreational Groups for Brain injury
survivors and program clients. These groups are funded primarily through grants
from the Statewide Head Injury Program, obtained by Community Rehab Care.
Community Rehab care employees also take an active
role in various events and groups supporting individuals with various
Neurological diagnosis; including Brain Injury
and stroke. Staff assist to lead the B.A.B.I.S. group ( Boston
acquired Brain Injury Support Group) and assist in planning the annual Heads Held High walk.

upon the activity

the flyers. All venues are handicap accessible.
