Why might I need a case manager?

A case manager offers assistance and support with things critical to maintaining your health and wellbeing. A case manager can help you overcome barriers to accessing services, set and achieve goals and find resources.

Will my case manager have experience working with a neurological diagnosis?

Yes, all the case managers at Community Rehab Care have experience working with individuals with a neurologic diagnosis. The case management team participates in ongoing trainings and educational opportunities in order to continue to learn and expand their knowledge.

Will my case manager be able to assist me plan for my future needs?

Yes, if future planning is a goal area for you, your case manager can assist you. For example, your case manager may assist you with finding legal services, finding housing resources, finding the appropriate medical supports, securing benefits, etc.

What if my family/support systems want to be involved in my care but they live out of state?

Your case manager will work with you and your family/support network no matter where they are located. Your family/support system will be included to the degree that you request.

When should I consider receiving case management services?

If you identify that you could benefit from help navigating resources, advocating for services, coordinating medical care, and ensuring you are safe in the community then the time is right to consider case management services. Many of the clients we service are receiving in home or outpatient therapy services (OT, PT, SP, nursing) while also receiving case management. The goal is to ensure you have a team of providers all focusing your goals and your health.

How and where are case management services delivered?

Case management services will be delivered through a combination of telehealth, in person visits, and phone calls. During COVID each client is reviewed to determine what is the safest and best practice for all involved.

Are Case Management services covered by insurance?

No, Case Management services outside of a facility are not recognized as a reimbursable service by insurance companies.

How do I start the process if I want to see if case management services are right for me?

Contact Community Rehab Care’s referral line.  A referral form will be completed over the phone.  Once the referral is completed the Case Management Supervisor will reach out to you. There is up to a 30-minute pro bono consultation allotted for this communication.  If you are interested in pursuing services, you will follow back with the referral line to set up a contract.

Is a case management assessment necessary?

Yes, the assessment allows the case manager to gain an understanding of the client’s level of functioning in the community, established linkages/resources, and future goals. The assessment includes a 1-hour meeting with the client/family. Write up of the assessment. Half hour meeting to review assessment and recommendations.

What happens after the case management assessment is completed?

The client/family will review the recommendations outlined in the assessment.  At that time, the client/family will decide if they want to move forward with case management services.  The client/family will also identify what recommendations they would like to address.  Recommendations can be tailored, and a plan established that works best for you financially and logistically.

Do I have to sign a contract?

Yes, a case management contract will be completed and signed. The contract will identify the staff who is providing the service. It will also outline to cost of the service and billing information (responsible party).

What will I be billed for?

In person meeting/appointments, all phone calls related to the case, researching/connecting with resources, completing applications, documentation, and team meetings.

Can I meet with my case manager to review my goals/plan at any point in time?

If you have questions or concerns about your goals/plan you should bring this to your case manager’s attention immediately.  If your goals/plan needs to be modified a meeting will be scheduled to review. The meeting will serve as a forum for everyone to be aware of the new plan moving forward.

Is there a requirement for how long I should engage in case management services for?

No, each case is unique and can be customized (no “cookie cutter” plans).
